Assistance in Coordinating or Managing Life Stages, Transitions and Supports

Mentoring and supporting individual skill development within the participant’s home or community For instance, assistance in attending appointments, shopping, bill paying, creating shopping list, taking part in social activities and maintaining contact with others.

Development of Daily Living and Life Skills

Individual life skills development and training including public transport training, attending to activities of daily living (cooking, cleaning, laundry, personal hygiene), developing skills for community, social and recreational participation.

Individual Social Skills Development

Assist participants to develop their social skills for participation in community and social activity by first assisting participants to understand themselves through exercise such as, understating their values, beliefs, hobbies/interest, personal goal, favourite foods/places, strengths and weaknesses. Then assist participants to find people that aligns with them.

Hourly rate - $74.63

Non-Face-to-Face Support Provision (Time spent writing reports, engaging with other professions and services providers, educating other support workers)

Hourly rate - $74.63/hr

(If 20min is spent for example speaking with another health profession, only 20min will be charged from the hourly rate.)

Provider travel

Up to 30min of travel to participant will be charged and 30min travel back to base

Hourly rate - $65/hr

(If 10min is spent travelling to the participant and 10min travel back, only 20min will be charged from the hourly rate. )
